Afya Jumuishi Project

The Afya Jumuishi Project is a national health initiative implemented by Management and Development for Health (MDH) with support from the U.S. Agency for International Development (USAID) under the PEPFAR program. “Afya Jumuishi,” meaning Integrated Health in Swahili, focuses on delivering comprehensive and integrated health services to ensure that communities across Tanzania receive high-quality, people-centered care.

The project aims to strengthen the delivery of HIV and TB services, family planning, maternal and child health (MCH), and gender-based violence (GBV) prevention. By working through regional and council health management teams, Afya Jumuishi enhances service quality, builds health-worker capacity, and promotes efficiency in health systems.

Through a network of implementing partners and local civil society organizations, Afya Jumuishi emphasizes inclusivity, community engagement, and sustainability—ensuring that every Tanzanian, especially those in underserved areas, has access to essential health services.

LHRO’s Participation in Afya Jumuishi

Life & Hope Rehabilitation Organization (LHRO) has proudly partnered in the Afya Jumuishi Project through a community-based grant aimed at reaching key and vulnerable populations in need of HIV and TB services.

As part of this collaboration, LHRO has played a vital role in connecting health facilities with hard-to-reach groups, particularly people who use and inject drugs (PWUIDs) and other at-risk populations. LHRO’s participation has focused on strengthening community systems, promoting integration between health and social services, and ensuring equitable access to care.

Through the Afya Jumuishi partnership, LHRO has:

  • Conducted HIV testing, counseling, and referral services in community settings.

  • Linked clients to treatment and care centers for HIV and TB management.

  • Provided psychosocial support, harm-reduction education, and rehabilitation services.

  • Trained peer educators and community health volunteers to sustain outreach efforts.

  • Collaborated with local health authorities to ensure smooth service delivery and follow-up.

Impact and Significance

LHRO’s engagement in the Afya Jumuishi Project has contributed significantly to the national goal of achieving integrated and inclusive health services. By combining MDH’s technical guidance with LHRO’s deep community networks, the partnership has improved access to HIV and TB services among key populations and strengthened linkages between community and facility care.

This collaboration demonstrates the power of partnership in transforming lives—promoting holistic, stigma-free, and sustainable health outcomes across Tanzania.
